Chronic care management process
Stage One
At this stage, we screen to determine whether the patient truly needs chronic care management based on eligibility criteria. Patients generally need chronic care management if they have two or more chronic illnesses expected to last 12 months or longer and if the conditions put them at significant risk.
Stage Two
At this stage, we educate the patient and obtain consent to enroll them in the chronic care management program. We explain the stages of the process, the importance of communication, the need for support and motivation, and finally the importance of consistent follow through with therapies, treatments, and all their condition requirements.
Stage Three
We conduct a thorough assessment and medication review. We check for drug interactions and ensure that the patient's various medications work well together and that negative side effects are minimized. We provide a carefully detailed care plan, and every person involved must have access to it. It is a complete plan of care for all health issues, paying the most attention to the chronic conditions.
Stage Four
After a care plan is in place, we keep track of the progress and give the patient complete access to our specialist and nurse team through a team member. This also allows us to stay in touch and be available at all hours every day for any consultation or to schedule appointments, keeping a continuation of the service.
Stage Five
At this stage, we monitor and assess to ensure that the patient's needs are being met. We continue to mange the patient's conditions, maintain documentation, and manage any transitions between facilities and providers to ensure continuity of care.
