What is the purpose of the 1823 & the 3008F Forms?
The 1823 form
The 1823 form is intended to determine whether assisted living care is appropriate for a patient. It must be completed by a licensed medical provider. The form requires many details on the patient’s condition, need for assistance, self care abilities, medical history, and more.
It also captures information regarding who is responsible for every part of the treatment process, daily living activities like eating, bathing, toileting, and more. All of this is essential information for the patient’s security and serves as legal documentation. This form must be updated at regular intervals as required by law or after any significant change in the patient's condition.
The 3008 form
The 3008 form is used to access Medicaid long term care services. It helps determine medical eligibility for Medicaid Waiver programs. This form must be signed by a licensed health care practitioner.
Patients Eligible for 1823 & 3008 forms must meet the Following requirements
Eligible Patients Must
Be at least 18 years of age.
Be free from signs and symptoms of any communicable disease.
Be able to perform the activities of daily living, with supervision or assistance if necessary.
Be able to transfer, with assistance if necessary. The assistance of more than one person is permitted.
Be capable of taking his/her own medication with assistance from staff if necessary
Eligible Patients Must NOT
Be a danger to self or others as determined by a physician.
Require licensed professional mental health treatment on a 24‑hour a day basis.
